Submit this online application with the required verification documents attached or visit the Transportation Center (located at 8800 Dorris Road, Douglasville GA) to present the required documents and have your photo taken for your ID card. You may also email documents or photo to: firstname.lastname@example.org .
If all components of the application (including attached documents and photo) are submitted online, Connect Douglas staff can print and mail your Reduced Fare badge to your home address.
Please allow up to 14 days for your application to be processed. After 14 days, please call Connect Douglas Customer Service at 770-949-7665 to check on the status of your application.
Reduced Fare Cards are not transferable and are only valid for the approved rider.
